Thank you for choosing to begin your therapy journey. This document contains important information about my professional services, policies, and your rights as a client. Please read it carefully and feel free to ask any questions. Once you feel informed and ready to proceed, kindly sign this consent form. Your signature confirms that you understand the nature of therapy and agree to the terms outlined below.
Therapist Information
I, Aditya Kumar, am a Counseling Psychologist committed to providing ethical, evidence-based, and client-centered care. I believe each individual is unique, and therefore I use an eclectic therapeutic approach, drawing from various evidence-based practices tailored to your needs, goals, and preferences.
Rights and Responsibilities
As a client, you have the right to:
- Ask questions at any point about the therapy process, techniques used, and expected outcomes.
- Decline or withdraw consent for any therapeutic intervention or technique.
- End therapy at any time without penalty or obligation to explain.
- Request a referral to another therapist if you wish to discontinue services with me.
- Receive respectful, non-judgmental, and confidential care.
As your therapist, I reserve the right to:
- Discontinue therapy in cases of non-compliance with therapy goals or disruptive behavior.
- Refer you to another professional if your needs fall outside my scope of practice or competence.
Confidentiality and Its Limits
All information shared in therapy is kept confidential and is protected under professional ethics and law. I will not disclose any information without your written consent, except under the following circumstances:
- If there is a threat of serious harm to yourself or others.
- Suspected abuse or neglect of a child, elderly person, or dependent adult.
- If I am required to comply with a court order or legal subpoena.
- As part of professional supervision (to ensure quality of care), where your identity will remain anonymous.
In all such cases, I will make every effort to inform you and involve you in the process where possible.
Session Structure and Frequency
- Sessions will last approximately 50–60 minutes.
- The frequency of sessions will be determined based on mutual agreement and the nature of your concerns.
- Therapy goals and focus areas will be collaboratively discussed and revised as needed.
Fees and Payment Policy
- The fee per session is: ₹2000. Students and individuals who are currently not working may opt for a sliding scale fee between ₹1000 to ₹1500, based on discussion and mutual agreement.
- All sessions must be paid for in advance.
- A receipt will be provided upon request.
Cancellation and No-Show Policy
- Clients may cancel or reschedule their session without charge up to 48 hours prior to the scheduled appointment time.
- Cancellations made within 24 hours of the scheduled session will incur a fee of 50% of the session charge.
- Cancellations made within 12 hours of the session, or failure to attend the session without prior notice (no-show), will incur the full session fee (100%).
- This policy ensures that reserved appointment times are respected and allows other clients the opportunity to access available slots. Genuine emergencies will be considered on a case-by-case basis at the therapist’s discretion.
Teletherapy Option
If sessions are conducted online (via video or phone), please ensure privacy at your location. In case of technical disruptions, the session may be rescheduled or continued via alternate communication.
Nature of Therapy
Therapy is a collaborative process and can bring about emotional insight, growth, and change. However, at times it may involve experiencing uncomfortable emotions. Please know that this is a normal part of the healing process.
There is no guarantee of specific outcomes, but your consistent participation and openness to the process significantly contribute to its effectiveness.
